Any person desiring a permit required by this division shall file with the sheriff
an application form which shall include the applicant's full name and any aliases
and his address, age, sex and fingerprints; the name, address and telephone number
of the applicant's employer, if any; and the location of the applicant's place of
business. Such application shall be accompanied by an application fee of two hundred
dollars ($200.00), which shall be deposited with the county treasurer.
